Schedule / Job Type

Contract | Days | Full-Time | Onsite | Monday–Friday, 8:30 AM–5:00 PM; one weekend per month may be requested; holiday work required with flexibility available upon request

Key Responsibilities

Provide physical therapy services in patients' homes, assisted living facilities, senior apartments, and board and care homes, seeing approximately 3–5 patients per day
Conduct Start of Care visits and evaluations, with visit weighting that accounts for travel, documentation, and case management time
Treat a varied patient population across diagnoses including orthopedic, cardiopulmonary, oncology, neurological, geriatric, and amputation cases (no pediatrics or hospice)
Supervise up to two PTAs or COTAs as part of the care team
Complete documentation and case management responsibilities within the scheduled shift, utilizing the EMR system
Participate in a structured three-week orientation process including online learning and field precepting prior to independent caseload management

Required Qualifications

Active California Physical Therapist (PT) license required
Basic Life Support (BLS) certification required
Active NPI number required
Minimum of 2 years of experience in a healthcare setting
Home Health experience including OASIS documentation required
Valid driver's license and proof of automobile insurance required

Preferred Experience / Skills

Experience with HomeCare HomeBase (HCHB) EMR system
Community or home health background strongly preferred
Ability to work effectively in varied home environments, including those that may be cramped, temperature-variable, or otherwise challenging
Strong oral and written communication skills with solid organizational ability
